Patient Outreach

Meadow Family Medicine runs frequent reports in an effort to ensure quality care. These reports are meant to capture the attention of members of our patient population who have not yet received various services, tests, screenings, vaccinations, etc. Our office staff places reminder calls, sends reminder letters, and utilizes the Patient Portal in an effort to provide quality care. We have included some of our outcomes on this website. Please take a moment to review our results. We hope this information explains why you may have experienced an increase in outreach attempts from this practice.

Colonoscopy Outreach

In May 2013 there were 258 patients on our list who had not completed colonoscopy screening. Of these 258 patients, 100% received outreach from this office requesting they complete this screening. Of these patients, 18 were compliant (7% completion rate).

In November 2013 there were 238 patients on our list who had not completed colonoscopy screening. Of that 238, 100% received outreach from this office requesting they complete this screening. Of these patients, 27 were compliant (11% completion rate).

The practice saw improvement in the completion rate in this area.

Mammogram Outreach

In July 2013 there were 185 patients on our list who had not completed mammography screening. Of these 185 patients, 100% of patients received outreach from this office requesting they complete screening. Of these patients, 92 were compliant (51% completion rate).

In July 2014 there were 219 patients on our list who had not completed mammography screening. Of that 219, 100% received outreach from this office requesting they complete screening. Of these patients, 70 were compliant (32% completion rate).

The practice did not see improvement in the completion rate in this area.

Chronic Care Outreach - Diabetes and Hyperlipidemia

In April 2013 there were 129 patients who had not completed lab work deemed necessary by our providers in the management of diabetes and hyperlipidemia. Of these 129 patients, 98% received outreach from this office requesting they complete lab work. Of these patients, 61 completed the requested lab work (48% completion rate).

In November 2013 there was a drop from 129 to 91 patients who had not completed lab work deemed necessary by our providers in the management of diabetes and hyperlipidemia. Of these 91 patients, 100% received outreach from this office requesting they complete lab work. Of these patients, 44 completed lab work (50% completion rate).

The practice saw improvement in the completion rate of this area.

Pneumovax Outreach (vaccination)

In September 2013 there were 178 patients on our list who had not received the pneumonia vaccination. Of these 178 patients, 100% received outreach from this office requesting they consider vaccination. Of these patients, 48 were vaccinated (27% completion rate).

In November 2013 there were 148 patients on our list who had not received the pneumonia vaccination. Of that 148, 100% received outreach. Of these patients, 15 were vaccinated (10% completion rate).

The practice did not see improvement in the completion rate in this area.

No-Appointment Patients in the Past 12 Months

In January 2013 there were 55 patients who had not been seen in the office in at least 12 months prior to the date that report was run. 100% of these patients received outreach. Of those 55 patients, 27 booked appointments in the office (20% completion rate).

In August 2013 there were 44 patients on this list. 100% of these patients received outreach. Of these 44 patients, 20 booked appointments in the office (45% completion rate).

The practice saw improvement in completion in this area.
